*Reaffirmation Period is June 10-14. For the requirements, kindly submit Certified True Copy of your Form 5 and one 1x1 ID picture. A brown envelope will be provided to put your forms into. Kindly affix your signature as well on the corresponding OSA forms for the organization's recognition.
*For those who will not reaff, kindly submit a letter of non-intent addressed to the Membership Committee.
